NYC Final Itinerary & Meeting Packet (Forms!)

Click the link below to access the complete packet of information distributed and discussed at the NYC Tour Choir Parent & Family Meeting on April 2nd.  All forms are due NO LATER THAN Thursday, April 11, 2019.  

Forms include:

• NHSD Overnight Field Trip Form (permission to attend & medical info)
• NHSD Overnight Medical Form (authorizing trip nurse to administer medication and/or giving permission to the student to self-administer)
• NHSD Social Media Form (giving chaperones permission to contact students by phone)
• NH Music Department Parent/Student Consent Agreement (behavior contract)
• Teacher Permission Slip for missing class on Monday, April 15

In addition, all traveling with the choir (students and adults) must complete the following Google Form with meal options for several of the scheduled meals.  Please complete this form ASAP so we can compile the orders and place them by Monday, April 8.
