Please use this page as a resource for festival information, forms, and links as we approach Honors Choir.  We're attempting to go more paperless this year, so some forms that have traditionally been mailed in will be linked as Google Forms.  Student contracts and medical forms must still be mailed in by USPS.

Student Forms

• Student Medical Form (print out and complete or type into file then print)
• Student Dietary Needs Form (online)
• PMEA Student Policy Acceptance Form (formerly the "Contract" or "Code of Conduct" forms - print out after completing)
• PMEA Medical Administration Record (to be completed for each medication that must be administered during the fest/festival)

Director Forms

• Medal order form (online)
• PMEA Principal Signature Page – This form is in place of requiring a principal’s signature for each student participant; rather all student participants in a festival must be listed on this form and then requires only one signature from the principal. The form must be completed and signed by the principal for EACH Fest/Festival and EACH advancement, if applicable (i.e. district, region, state)
