Student contracts and medical forms must still be mailed in by USPS.

- PMEA Principal Signature Page – This form is in place of requiring a principal’s signature for each student participant; rather all student participants in a festival must be listed on this form and then requires only one signature from the principal. The form must be completed and signed by the principal for EACH Fest/Festival and EACH advancement, if applicable (i.e. district, region, state)
